Our Single Point of Access (SPOA) helps adults and children with serious mental health needs connect to the right services and supports. We work with local providers to match individuals to care that promotes independence, improves quality of life and honors personal choices.
Children’s SPOA (C-SPOA)
About C-SPOA
The Purpose of Children’s SPOA:
- Identify children with the highest risk for placement
- Develop strategies to manage these children in their home communities
- Develop better decisions about individualized care planning for children at risk
- Support communities to manage access to intensive services
The Children’s SPOA aims to ensure timely and appropriate access to intensive services for children at risk of multiple hospitalizations and out-of-home treatment. The C-SPOA Team provides case consultation, systems navigation, referral and linkage to mental health supports and services.

How to Apply for C-SPOA – Video Tutorial from NYS Office of Mental Health:
Understanding the Application Process:
C-SPOA Application Part 1 (Pages 1-8 only):
For referring a youth and their family for C-SPOA coordination. C-SPOA Coordinators help coordinate access to services and supports for youth with unmet mental health needs. This application asks for general information about a youth’s demographics and mental health needs.
C-SPOA Application Part 2 (Pages 1-17 required):
For accessing Youth Assertive Community Treatment (ACT), Children’s Community Residence (CCR), and Residential Treatment Facility (RTF) services. This application requests more detailed information around a youth’s mental health needs and provides the requirements for documentation to support any needs noted.
Adult SPOA (A-SPOA)
About A-SPOA
Eligibility Criteria:
- Adults age 18 or older
- Current Lewis County resident or planning to relocate to Lewis County
- Diagnosed with a Serious Mental Illness (SMI)
- Experiencing functional impairments due to mental illness
- Need for residential supports, case management, Assisted Outpatient Treatment (AOT) or an Enhanced Voluntary Agreement (EVA).
Required Documentation:
- Psychiatric Assessment with ICD-10 codes/DSM V Diagnosis Codes
- Comprehensive Assessment
- Current Medication List
- Physician’s Authorization (OMH – CR and ATP/TAP residential placement only)
- Hospital Admission/Discharge Summaries (if available)
