Recently, there have been changes to the ACA Reporting Requirements. In the past, Forms 1095-C were distributed each year to verify your health insurance participation.
  • Due to newly enacted Federal Legislation, Lewis County is no longer required to send Form 1095-Cs to individuals unless a form is requested. If you request a form 1095, your request will be fulfilled within 30 days of the request. If you would like a copy of your 1095-C Statement, please send your request to:

Here are the new rates for you and your spouse (if both Medicare Eligible):

Hospital Retirees hired prior to 07/01/1990 or County Retirees hired prior to 01/01/1991
  • Individuals:                         $0.00/month             $0.00/quarter
  • Retiree & Spouse:             $132.08/month         $396.24/quarter
  • Retiree & Family:             $198.12/month         $594.36/ quarter
Hospital Retirees hired on or after 07/01/1990 or County Retirees hired on or after 01/01/1991
  • Individuals:                         $127.00/month         $ 381.00/quarter
  • Retiree & Spouse:             $254.00/month         $ 762.00/quarter
  • Retiree & Family:             $381.00/month         $1,143.00/ quarter
Please call if you or your spouse is on the Medicare Advantage Plan and the other is on the Lewis County Health Plan.
